Licorice root (Glycyrrhiza glabra) has a long history of use in traditional medicine systems such as Traditional Chinese Medicine and Ayurveda for soothing sore throats and coughs. More recently, scientific studies have begun to elucidate its mechanisms and efficacy for this purpose. The root contains compounds like glycyrrhizin and flavonoids, which exhibit anti-inflammatory, demulcent (soothing), and antimicrobial properties. Several clinical trials have specifically investigated the use of licorice root gargles in patients undergoing intubation or throat surgery. These studies found that patients using licorice root solutions experienced significantly less postoperative sore throat compared to placebo groups.

In addition to randomized controlled trials, in vitro experiments demonstrate that licorice root extracts can inhibit the growth of certain bacteria and viruses known to cause throat infections. The anti-inflammatory effects may reduce swelling and pain in the throat mucosa, and its demulcent action helps coat and soothe irritated tissues.

However, while the evidence is promising, most clinical trials have focused on licorice root used as a gargle rather than an oral supplement, and many studies are relatively small. There are also safety concerns with excessive or prolonged use, such as hypertension and electrolyte imbalances due to glycyrrhizin. Thus, licorice root’s use for sore throat is supported by moderate scientific evidence (rated 3/5), particularly for short-term, topical applications.
