Magnesium has scientific support for its use in certain types of chest pain, particularly those associated with acute myocardial infarction (heart attack) and certain arrhythmias, but its use is not universal across all forms of chest pain. Magnesium is an essential mineral that plays a critical role in cardiovascular health, including vascular tone, heart rhythm, and platelet function. Clinical studies, including randomized controlled trials, have explored intravenous magnesium sulfate as an adjunctive treatment in acute myocardial infarction. Meta-analyses have shown that while early studies suggested a reduction in mortality, more recent large-scale trials (such as ISIS-4 and MAGIC) have provided mixed results, leading to uncertainty about routine use in this setting. The American Heart Association and other major guidelines do not recommend magnesium for all patients with chest pain or heart attack, but it may be considered in those with documented hypomagnesemia or certain arrhythmias (such as torsades de pointes) that can present as chest discomfort or palpitations. Additionally, magnesium can relieve coronary artery spasm, which may help in variant (Prinzmetal’s) angina, though evidence is limited. In summary, while there is scientific rationale and some clinical evidence for magnesium’s use in specific cardiac contexts, its role in the general management of chest pain is limited and not broadly endorsed by guidelines.
