Folate (vitamin B9) has a modest scientific basis for use in supporting or treating canker sores, also known as recurrent aphthous stomatitis (RAS). Several observational studies have found associations between deficiencies in folate (as well as other B vitamins, iron, and zinc) and an increased risk of developing canker sores. Some small-scale clinical trials and case reports suggest that supplementation with folic acid may help reduce the frequency or severity of canker sores, particularly in individuals who are deficient in this vitamin. For example, a randomized controlled trial published in the Journal of Oral Pathology & Medicine (2009) found that patients with RAS who received folic acid supplementation showed improvement compared to placebo, though the sample size was small and results should be interpreted cautiously.

However, the overall quality of evidence is limited. Most studies are small, and not all people with canker sores have folate deficiency. The proposed mechanism is that folate plays a role in maintaining healthy mucosal cells and in DNA synthesis and repair, which may be important for oral tissue integrity. As a result, some clinicians may recommend folate supplementation for individuals with frequent canker sores, particularly if a deficiency is identified. Nonetheless, routine folate supplementation for all cases of canker sores is not universally endorsed by clinical guidelines, and more robust research is needed to clarify its effectiveness.
