Menthol oil is commonly used in topical preparations for backache and other musculoskeletal pains. Its use is primarily supported by scientific evidence, though the quality and quantity of that evidence are moderate. Menthol, the active component in menthol oil, acts as a counterirritant. When applied to the skin, it stimulates cold receptors, producing a cooling sensation that helps distract from deeper underlying pain. This mechanism is known as "gating" or the "gate control theory" of pain, where sensory input from menthol competes with pain signals, thereby reducing the perception of pain.
Several clinical studies and reviews support the use of topical menthol for temporary relief of minor aches and pains associated with backache, arthritis, strains, and sprains. For example, a 2014 review published in the journal Current Medical Research and Opinion found that topical menthol preparations provided moderate pain relief for musculoskeletal conditions, including low back pain. However, these effects are generally short-lived, and menthol is not considered a treatment for the underlying causes of back pain.
While menthol oil is widely used and generally recognized as safe for topical application, the scientific evidence rates at about a 2 out of 5, indicating modest support, mainly for symptomatic relief.
