Magnesium has a scientific basis for use as an adjunctive treatment in severe tetanus, particularly in cases of autonomic dysfunction. Tetanus, caused by the neurotoxin tetanospasmin from Clostridium tetani, leads to muscle rigidity, spasms, and dangerous autonomic instability (e.g., hypertension, tachycardia). Magnesium sulfate acts as a neuromuscular blocker and stabilizes the autonomic nervous system by inhibiting catecholamine release and blocking calcium influx into neurons, thereby reducing muscle spasm severity and controlling cardiovascular instability.

Clinical evidence, while limited, is supportive. A landmark randomized controlled trial by Thwaites et al. (2006, NEJM) involving adults with severe tetanus demonstrated that magnesium sulfate infusion was safe and effective in reducing the need for other drugs (such as sedatives and muscle relaxants) to control spasms and autonomic symptoms, although it did not significantly reduce mortality. Systematic reviews and case series also suggest magnesium can be a valuable adjunct, especially where mechanical ventilation is available.

While not a cure or replacement for standard therapies (antitoxin, antibiotics, wound care, and supportive measures), magnesium’s role is increasingly recognized in modern intensive care settings, particularly in resource-rich environments. Thus, while more high-powered studies could further clarify its benefits, current evidence supports magnesium sulfate as a scientifically justified adjunct in managing severe tetanus.
