Ligusticum wallichii (commonly known as Chuanxiong or Szechuan lovage) is a traditional Chinese medicinal herb widely used in East Asian medicine. Its primary historical use is for promoting blood circulation, dispelling wind, and alleviating pain, especially in the context of headaches, menstrual disorders, and cardiovascular issues. However, traditional Chinese medicine (TCM) formulations sometimes include Ligusticum wallichii in herbal combinations intended to treat respiratory ailments, including pneumonia, due to its purported abilities to "invigorate the blood" and potentially reduce inflammation.

Despite its inclusion in some traditional prescriptions for pneumonia, there is limited direct scientific evidence to support its efficacy specifically for this condition. Most modern pharmacological research on Ligusticum wallichii focuses on its anti-inflammatory, antioxidant, and neuroprotective properties, as well as its effects on cardiovascular health. Some studies have suggested that its active components, such as ligustilide and ferulic acid, may have anti-inflammatory and vasodilatory effects, which could theoretically benefit respiratory conditions characterized by inflammation. Nevertheless, these findings do not directly validate its use for pneumonia treatment, nor do they establish clear antimicrobial or direct anti-pneumonic activity. Thus, current support for using Ligusticum wallichii in pneumonia is rooted more in traditional practice than in robust scientific validation.
