Flaxseed (Linum usitatissimum) is sometimes used as an adjunctive therapy for Inflammatory Bowel Disorders (IBD), including Crohn’s disease and ulcerative colitis. The scientific rationale for its use is based on both preclinical and limited clinical evidence suggesting anti-inflammatory and mucosal barrier-supporting effects. Flaxseed is rich in alpha-linolenic acid (ALA), a plant-based omega-3 fatty acid, and lignans, which have demonstrated anti-inflammatory properties in animal models of colitis. Studies in rodents have shown that flaxseed supplementation can reduce colonic inflammation, oxidative stress, and improve histological markers of IBD. Limited human evidence exists. A few small clinical trials and observational studies have explored flaxseed’s effects on gastrointestinal health, but robust, well-powered trials specifically in IBD patients are lacking. Some data suggest possible symptom relief and reduced inflammatory markers; however, these findings are preliminary and not universally replicated. Mechanistically, flaxseed may modulate gut microbiota and produce short-chain fatty acids (SCFAs), which are beneficial for gut integrity. In summary, while there is a plausible scientific basis and some promising preclinical results, clinical evidence remains insufficient to firmly recommend flaxseed as a primary therapy for IBD. Patients should consult healthcare providers before using flaxseed therapeutically, especially due to its high fiber content, which may not be suitable during active disease flares.
