American Basil (Ocimum americanum), also known as hoary basil or wild basil, has a history of use in traditional medicine systems, particularly in parts of Africa and Asia. Folk remedies have employed basil species—including American Basil—for the relief of headaches, especially those associated with sinus congestion. The aromatic leaves are often used in infusions, decoctions, or as steam inhalations, based on the belief that their volatile oils can help open nasal passages and relieve pressure. The use is primarily justified by tradition, as these practices have been passed down through generations.

However, the direct scientific evidence supporting American Basil’s efficacy specifically for sinus headache is limited. Some studies on related basil species (such as Ocimum basilicum) suggest mild anti-inflammatory, analgesic, and decongestant properties, attributed to compounds like eugenol, linalool, and camphor found in their essential oils. These findings provide a plausible mechanism but are not specific to American Basil or to sinus headaches. Clinical trials and high-quality studies on humans are lacking. Therefore, while there is a traditional basis for its use in sinus headache, scientific validation is minimal, and the evidence strength is rated low.
