Salicin is a natural compound found primarily in the bark of willow trees (Salix species). Its traditional use dates back thousands of years, as ancient civilizations, including the Greeks and Native Americans, used willow bark extracts to relieve pain and reduce fever. The justification for its use in headache relief, however, is not solely traditional: scientific validation exists. Salicin is metabolized in the body to salicylic acid, which is the precursor to acetylsalicylic acid—commonly known as aspirin.

Modern pharmacological studies have demonstrated that salicin and its derivatives exert anti-inflammatory and analgesic effects, mediated primarily by the inhibition of cyclooxygenase (COX) enzymes and the subsequent reduction in prostaglandin synthesis. Randomized controlled trials have shown that willow bark extracts can reduce pain in conditions such as osteoarthritis and lower back pain. While fewer studies have focused specifically on headaches, the mechanism of action is similar to that of aspirin, which is well-established for headache relief (including tension-type and migraine headaches).

Despite this, the evidence base for salicin in headache treatment is not as robust or direct as for aspirin, and most clinical trials have studied pain relief in other contexts. Nonetheless, given its pharmacological similarity to aspirin, the use of salicin-containing willow bark for general headache relief is supported by a moderate level of scientific evidence, albeit with less direct clinical trial support specific to headaches themselves.
