Folk and herbal medicine systems, including those in parts of Asia and the Americas, have used capsicum for headache relief, often as poultices or ointments applied directly to the skin, especially on the forehead or temples.

There is some emerging scientific evidence, primarily relating to topical capsaicin creams or nasal sprays, that supports its use for certain types of headaches, especially cluster headaches. Small clinical trials and case series have demonstrated that intranasal capsaicin can reduce the frequency or intensity of cluster headaches in some patients, possibly by desensitizing sensory neurons.
