Proanthocyanidins are a class of polyphenolic compounds found abundantly in foods such as grapes, cranberries, and certain types of bark. There is growing scientific interest in their use for oral health, particularly in relation to gingivitis, due to their antioxidant, anti-inflammatory, and antimicrobial properties. Several in vitro and animal studies have demonstrated that proanthocyanidins can inhibit the adhesion and growth of key periodontal pathogens such as Porphyromonas gingivalis and Streptococcus mutans. These bacteria are known contributors to plaque formation and gingival inflammation. Some small-scale human clinical trials and pilot studies have indicated that products containing proanthocyanidins (especially from cranberries) may reduce plaque accumulation and gingival bleeding. However, the number of robust, large-scale randomized controlled trials is limited, and results are sometimes inconsistent. Most available evidence suggests a modest benefit, likely due to a reduction in bacterial colonization and modulation of the host inflammatory response. Systematic reviews to date suggest that while promising, the quality and quantity of clinical evidence is insufficient to make strong recommendations for their routine use in managing gingivitis. In summary, there is preliminary scientific evidence, mostly from laboratory and small human studies, supporting the use of proanthocyanidins for gingivitis, but more rigorous clinical research is needed to confirm efficacy and determine optimal dosing and delivery methods.
