Black garlic, a fermented form of fresh garlic (Allium sativum), has gained attention for its enhanced antioxidant content and unique bioactive compounds, including S-allyl cysteine. Its use in supporting or treating gingivitis is primarily justified by emerging scientific evidence, though the strength of this evidence remains moderate (rated 2/5). Several in vitro and animal studies have demonstrated that black garlic extracts possess significant antibacterial activity against oral pathogens such as Porphyromonas gingivalis, a key bacterium implicated in the development of gingivitis and periodontitis. Furthermore, black garlic exhibits anti-inflammatory and antioxidant properties, which may help reduce gum inflammation and oxidative stress in the oral cavity.

A few small-scale clinical studies and pilot trials suggest that black garlic mouthwashes or gels can reduce plaque accumulation and gingival bleeding scores compared to controls, though these studies often have limitations such as small sample sizes and short durations. While these initial findings are promising, there is a lack of large, high-quality randomized controlled trials specifically evaluating black garlic for gingivitis in humans.
