Lactobacillus acidophilus is a probiotic bacterium commonly used in both dietary supplements and fermented foods. Its use for gastritis is supported by some scientific evidence, although the strength of that evidence is currently limited, meriting a rating of 2 out of 5. Gastritis, defined as inflammation of the gastric mucosa, can be caused by infection (especially Helicobacter pylori), NSAID use, alcohol, or other irritants. Several studies and reviews have explored the use of probiotics, including L. acidophilus, in managing gastritis, especially for H. pylori-associated cases. Some randomized controlled trials have found that supplementation with L. acidophilus can help reduce the bacterial load of H. pylori and may alleviate some gastritis symptoms when used alongside standard antibiotic therapy. Mechanisms proposed include production of lactic acid (which can inhibit H. pylori), enhancement of mucosal barrier function, and modulation of immune responses.

However, most studies employ probiotic mixtures rather than L. acidophilus alone, and the effects appear modest. Evidence for benefit in non-H. pylori gastritis is weaker still. There is little historical or traditional use of L. acidophilus specifically for gastritis, with most use being recent and based on evolving knowledge of the gut microbiome. Overall, L. acidophilus may provide some adjunctive benefit for gastritis, particularly in cases associated with H. pylori, but more robust, high-quality clinical trials are needed to confirm its efficacy and clarify its specific role.
