Terpenoids, a large class of naturally occurring organic chemicals derived from terpenes, have a longstanding history of use in traditional medicine for various skin conditions, including eczema. Many plant essential oils and extracts that are traditionally applied to eczematous skin contain terpenoids, such as those from chamomile, tea tree, and lavender. These preparations have been used historically to soothe itching, reduce inflammation, and promote healing, effects attributed to their terpenoid constituents. However, while there are some in vitro and animal studies suggesting that certain terpenoids possess anti-inflammatory and antimicrobial properties, robust clinical trials in humans with eczema are limited or lacking for most individual terpenoids or for terpenoid mixtures. The traditional use is thus based on empirical experience rather than strong scientific validation. Some terpenoids, like beta-caryophyllene and linalool, have shown potential in preclinical studies, but their efficacy and safety for eczema in humans remain to be established. Overall, the use of “terpenoid (unspecified)” for eczema is primarily justified by traditional practice, with only weak scientific support as of now.
