Salvia, a genus that includes many species such as Salvia officinalis (common sage) and Salvia miltiorrhiza (Danshen), has a long history of use in traditional medicine systems for various skin conditions, including eczema. The traditional use is most prominent in Chinese and Western herbal medicine, where sage leaves or extracts have been applied topically or consumed to address skin inflammation and promote healing. The presumed basis for this use is the herb’s anti-inflammatory and antimicrobial properties, which have been described anecdotally or in historical texts. However, scientific evidence directly supporting the use of Salvia species for eczema is limited. A few laboratory studies suggest that certain compounds in Salvia, such as rosmarinic acid and diterpenoids, may have anti-inflammatory effects that could theoretically benefit eczema. Nonetheless, robust clinical trials in humans with eczema are lacking. As a result, Salvia’s use for eczema is justified primarily by tradition, with only weak preliminary biological plausibility from in vitro or animal research. No major clinical guidelines recommend Salvia for eczema, and scientific validation remains insufficient at this time.
