Bifidobacteria are a genus of beneficial gut bacteria that have been investigated for their potential role in supporting skin health, including the management of atopic dermatitis (eczema). The scientific rationale for their use centers on the "gut-skin axis," a concept suggesting that gut microbiota can influence systemic inflammation and immune responses, which are relevant in eczema.

Several randomized controlled trials and meta-analyses have explored the effects of probiotic supplementation (often including Bifidobacteria species such as Bifidobacterium lactis and Bifidobacterium breve) in children and infants with eczema. Some studies have reported modest improvements in eczema severity scores and reduced risk of developing eczema, particularly when administered prenatally to mothers and postnatally to infants. However, results are inconsistent, with other studies showing little or no benefit compared to placebo. The mechanisms proposed include enhancement of the intestinal barrier, modulation of immune responses (e.g., shifting the Th1/Th2 balance), and reduction of systemic inflammation.

Systematic reviews, including a 2018 Cochrane review, conclude that while probiotics (including Bifidobacteria) may offer a small benefit in preventing eczema in high-risk infants, evidence for their use in treating established eczema is limited and of low certainty. Thus, the use of Bifidobacteria for eczema is supported by some scientific evidence, but this evidence is not strong or conclusive. Further high-quality research is needed to determine which strains, doses, and patient populations might benefit most.
