Green tea, specifically its extract containing epigallocatechin gallate (EGCG), has attracted scientific interest for potential adjunctive treatment in Down syndrome (DS). The rationale stems from the fact that individuals with DS have an extra copy of chromosome 21, which includes the gene DYRK1A. Overexpression of DYRK1A is implicated in cognitive deficits observed in DS. EGCG, a prominent catechin in green tea, has been shown in preclinical studies to inhibit DYRK1A activity. Several animal studies have demonstrated that EGCG can partially rescue cognitive performance and synaptic plasticity in Down syndrome mouse models. These promising results led to small-scale clinical trials in humans. The most notable is a randomized, double-blind, placebo-controlled trial published in Lancet Neurology (2016), which found that a 3-month course of EGCG, combined with cognitive training, produced modest improvements in some cognitive domains in young adults with DS. However, the effects were not uniformly robust, and some benefits waned after discontinuation. Safety concerns, especially regarding liver toxicity at high doses, have also been raised. Importantly, while the scientific rationale and early results are promising, evidence is still preliminary and not sufficient to recommend green tea or EGCG as a standard treatment for DS. Larger, longer-term trials are needed to confirm efficacy and safety. Therefore, the evidence rating is moderate-low, reflecting cautious optimism but an incomplete evidence base.
