Bifidobacterium species are among the most widely studied probiotics for the management of diarrhea, especially in children and in cases of antibiotic-associated diarrhea. Numerous randomized controlled trials and meta-analyses have demonstrated that certain strains of Bifidobacterium (such as B. bifidum, B. infantis, and B. longum) can help reduce the duration and severity of acute infectious diarrhea and antibiotic-associated diarrhea. The mechanisms proposed include restoration of normal gut flora, strengthening of the intestinal barrier, competition with pathogens, and modulation of the host immune response.
The World Gastroenterology Organisation and ESPGHAN (European Society for Paediatric Gastroenterology Hepatology and Nutrition) have recognized the use of certain probiotics, including Bifidobacterium, as effective adjuncts in the management of acute diarrhea in children. A 2019 Cochrane review found that probiotics, including Bifidobacterium, shorten the duration of diarrhea by about 24 hours in pediatric populations. Evidence for adults is somewhat less robust, but beneficial effects have also been reported, especially when Bifidobacterium is part of a multi-strain probiotic product.
It is important to note that efficacy is strain-specific and not all products labeled as "Bifidobacterium" will be equally effective. Nonetheless, there is moderate to strong scientific support for the use of well-studied Bifidobacterium strains in the supportive treatment of diarrhea, leading to a rating of 4 out of 5 for evidence.
