Eicosapentaenoic acid (EPA) is an omega-3 fatty acid found primarily in fish oil. Its use for supporting or treating depression is backed by a growing body of scientific evidence, rather than traditional use. Several randomized controlled trials and meta-analyses have investigated the effects of omega-3 fatty acids, particularly EPA, on depressive symptoms. Results suggest that EPA, especially at higher ratios compared to docosahexaenoic acid (DHA), may have a modest but statistically significant benefit in reducing depressive symptoms, particularly in individuals with major depressive disorder (MDD) who are not responding adequately to standard antidepressant therapies. The proposed biological mechanisms include EPA’s role in modulating inflammation, neurotransmitter function, and cell membrane fluidity in the brain, all of which may influence mood regulation. Notably, some studies indicate that EPA-predominant formulations (with at least 60% EPA) are more effective than DHA or mixed formulations. However, the magnitude of benefit is moderate, and not all studies have found positive effects, leading to ongoing debate about the strength of the evidence. Guidelines from some psychiatric and nutritional organizations acknowledge the potential adjunctive benefit of omega-3s, particularly EPA, for depression, but emphasize that they should not replace standard antidepressant treatments. Thus, while the evidence is not definitive, there is scientific validation supporting the adjunctive use of EPA in the management of depression.
