Ligustilide is a phthalide compound found primarily in plants such as Angelica sinensis ("Dong Quai") and Ligusticum chuanxiong, both of which are widely used in traditional Chinese medicine (TCM). In TCM, these herbs are often prescribed for their supposed effects on promoting blood circulation and alleviating various ailments, including constipation. The use of ligustilide-containing herbs for constipation, including in children, is based on traditional practices rather than robust scientific validation. There are some preclinical studies indicating that ligustilide may have smooth muscle relaxant effects, which could theoretically influence intestinal motility. However, direct evidence—especially well-controlled clinical trials—supporting the efficacy and safety of ligustilide or its source herbs for treating constipation in children is lacking. The majority of references to its use for constipation are rooted in historical TCM texts and anecdotal practitioner reports. As such, while there is a traditional precedent for using ligustilide-containing herbs for constipation, the scientific evidence base remains minimal, and its use should be approached with caution, particularly in pediatric populations.
