Isomalto-oligosaccharide (IMO) is a type of prebiotic dietary fiber commonly added to foods and supplements. Its use for supporting or treating constipation, including in children, has a basis in scientific research. IMOs are not significantly digested in the small intestine; instead, they reach the colon, where they are fermented by beneficial gut bacteria. This fermentation process increases the production of short-chain fatty acids, which can stimulate bowel movements and improve stool consistency.
Several clinical studies have explored the effects of IMO and other prebiotics on bowel function in children. For example, randomized controlled trials have shown that prebiotics such as IMOs can increase stool frequency and improve stool softness in children with functional constipation, though the effect size is generally modest. The mechanism is thought to involve both increasing fecal bulk and enhancing beneficial bacterial populations, such as Bifidobacteria, which are associated with improved gut motility.
However, while there is scientific support for the use of IMOs, the evidence is not as robust or consistent as it is for other fiber sources such as inulin or certain oligosaccharides (e.g., GOS or FOS). The overall rating is moderate (3/5) because, although studies have shown benefit, the number and quality of trials specifically in children are limited, and some studies use mixtures of prebiotics rather than IMO alone. Nonetheless, IMO is regarded as a safe and potentially beneficial adjunct for managing mild pediatric constipation.
