Fructo-oligosaccharides (FOS) are short-chain carbohydrates classified as prebiotic fibers. Their use in the management of constipation in children is primarily supported by scientific evidence. FOS are not digested in the upper gastrointestinal tract; instead, they reach the colon where they are fermented by beneficial gut bacteria, leading to increased production of short-chain fatty acids. This fermentation process increases stool bulk and water content, which helps stimulate bowel movements and alleviate constipation.

Several randomized controlled trials have evaluated the efficacy of FOS in treating pediatric constipation. For example, a double-blind, placebo-controlled study published in Pediatrics International (2014) found that children who received FOS experienced significant improvements in stool frequency and consistency compared to placebo. FOS is generally well-tolerated in pediatric populations, with mild gastrointestinal symptoms (such as flatulence or bloating) being the most commonly reported side effects. Systematic reviews and meta-analyses have concluded that prebiotic fibers like FOS can be a useful adjunct in managing mild to moderate childhood constipation, though they are not a substitute for other medical interventions in severe cases.
