Bifidobacterium species are probiotics that have been investigated for their role in alleviating constipation, including in children. Scientific interest is based on the observation that children with functional constipation often have altered gut microbiota, with lower levels of bifidobacteria. Several randomized controlled trials (RCTs) and systematic reviews have examined the efficacy of Bifidobacterium strains—most commonly B. lactis and B. longum—either alone or in combination with other probiotics, in improving stool frequency and consistency in constipated children.
Evidence suggests a moderate benefit for certain strains. For example, supplementation with B. lactis DN-173 010 has shown to increase stool frequency and soften stools in children with constipation in some studies, though not all outcomes reach statistical significance. Systematic reviews (e.g., by the ESPGHAN and NASPGHAN working group) conclude that while some studies demonstrate benefit, overall evidence remains limited by small sample sizes, variability in strains and dosages, and methodological heterogeneity. Probiotics are generally safe and well-tolerated in children.
In summary, the use of Bifidobacterium for pediatric constipation is supported by moderate scientific evidence, particularly for select strains, but is not yet established as a standard treatment. Larger, high-quality trials are needed for clear recommendations.
