Lactobacillus casei is a probiotic bacterium that has been studied for its potential benefits in supporting gut health, including the management of constipation in adults. The rationale for its use is based on its ability to help restore and maintain a healthy balance of intestinal microbiota, which can influence bowel regularity and stool consistency. Several randomized controlled trials and systematic reviews have evaluated the efficacy of L. casei for constipation, particularly in populations with chronic functional constipation.
Some studies, such as a 2014 randomized, double-blind, placebo-controlled trial published in the journal Beneficial Microbes, found that daily supplementation with L. casei Shirota improved stool frequency and consistency in adults with constipation. Meta-analyses and systematic reviews have generally concluded that certain probiotic strains, including L. casei, may provide modest benefits in increasing stool frequency and improving overall symptoms of constipation compared to placebo. The effects, however, are often strain-specific and not all studies have reported significant results.
Despite some positive findings, the overall quality of evidence is considered low to moderate due to variability in study design, probiotic dosage, and participant populations. As a result, clinical guidelines generally recommend probiotics as a supplementary option rather than a primary treatment for constipation.
