Echinacea has a long history of traditional use among Native American tribes and in Western herbal medicine, primarily as a remedy for respiratory tract infections and symptoms such as congestion. The root and aerial parts of Echinacea purpurea, Echinacea angustifolia, and other species have been used in teas, tinctures, and extracts to help alleviate symptoms of the common cold and sinus congestion. The rationale behind its traditional use is based on its perceived ability to stimulate the immune system and reduce the severity or duration of respiratory symptoms, including nasal and sinus congestion.

However, scientific evidence specifically supporting Echinacea for treating or relieving sinus congestion is limited and mixed. Most clinical studies have focused on its potential to reduce the duration and severity of the common cold, and results are inconsistent. Some studies suggest a modest benefit, while others find no significant effect. There is little direct evidence from controlled trials that Echinacea effectively reduces sinus congestion specifically. Thus, its use for this purpose is primarily rooted in tradition rather than strong scientific validation.
