Belleric myrobalan (Terminalia bellirica), known as "Bibhitaki" in Ayurveda, has a long-standing traditional use in Indian medicine for respiratory conditions, including sinus congestion. In Ayurvedic texts, Bibhitaki is described as having expectorant properties and is a key ingredient in the classical formulation "Triphala." It is traditionally believed to support respiratory health by helping to clear excess mucus and promote the healthy functioning of the sinuses and airways. The fruit is said to balance the "Kapha" dosha, which is commonly associated with mucus and congestion in Ayurvedic theory.

Despite these traditional uses, there is limited direct scientific research specifically validating the efficacy of Belleric myrobalan for sinus congestion in humans. Some in vitro and animal studies have demonstrated anti-inflammatory, antimicrobial, and antioxidant activities of the fruit extracts, which could theoretically support its use in respiratory conditions, but clinical research is lacking. No robust, well-controlled human trials have established its direct benefit for sinus congestion or related symptoms. Thus, while it remains a popular remedy in traditional systems, the evidence supporting its use for sinus congestion is primarily traditional, with modest support from preclinical studies.
