Sichuan pepper (Zanthoxylum species) has a long history of use in Traditional Chinese Medicine (TCM), where it is known as "huā jiāo" (花椒). In TCM, it is classified as acrid and warm, and is traditionally used to dispel cold, warm the middle burner (digestive system), and alleviate pain. While its primary applications are more commonly associated with gastrointestinal complaints and pain relief, historical texts do mention its use in treating various forms of "phlegm" and "dampness," which can be interpreted as conditions involving congestion, including in the lungs. The warming and dispersing qualities are believed to help "open the orifices" and facilitate the movement of qi, potentially aiding in the resolution of phlegm and congestion. However, there is very limited modern scientific research validating these traditional uses for lung congestion specifically. Laboratory studies on Sichuan pepper have mainly focused on its antimicrobial, anti-inflammatory, and analgesic properties, rather than direct effects on respiratory congestion. There is a lack of clinical trials or pharmacological studies demonstrating clear efficacy for lung congestion. Therefore, the use of Sichuan pepper for lung congestion is justified mainly by tradition, with some theoretical support from TCM principles, but with little direct scientific evidence.
