Atractylodes, particularly Atractylodes macrocephala and Atractylodes lancea, has a long history of use in Traditional Chinese Medicine (TCM). It is known as "Bai Zhu" or "Cang Zhu" and is primarily used to strengthen the spleen, dry dampness, and promote water metabolism. In the context of treating bronchial congestion, its use is more indirect. TCM theory posits that when the spleen is weak and dampness accumulates, this can lead to phlegm formation, which may manifest as bronchial congestion. Atractylodes is thus incorporated into herbal formulas to "transform phlegm" and "dry dampness," thereby supporting respiratory health. However, modern scientific studies specifically evaluating atractylodes for bronchial congestion are limited. There is some preliminary evidence that extracts from atractylodes possess anti-inflammatory and immunomodulatory properties, which could theoretically benefit respiratory conditions, but direct clinical evidence for its efficacy in treating bronchial congestion is lacking. Most of the justification for its use comes from centuries of traditional use and inclusion in classic TCM formulas for phlegm-related respiratory symptoms. Overall, while the traditional rationale is strong and there is plausible biological activity, the direct scientific validation for this specific indication is currently weak.
