Zinc has been investigated as a treatment for cold sores (herpes labialis), primarily due to its antiviral and immune-modulating properties. Several topical zinc formulations, such as zinc oxide or zinc sulfate creams, have been evaluated in clinical studies for their ability to reduce the duration and severity of cold sore outbreaks. The rationale is that zinc may inhibit the replication of herpes simplex virus (HSV-1), the primary cause of cold sores, and promote faster healing of lesions.

A few randomized controlled trials have demonstrated modest benefits. For example, one study published in Alternative Therapies in Health and Medicine (2001) found that a topical zinc oxide/glycine cream shortened the duration of cold sore episodes compared to placebo. Another study in Medical Microbiology and Immunology (2000) showed that zinc sulfate solution reduced symptom severity, though results have varied between studies, and some have reported only minimal effects.

Overall, while there is some scientific evidence supporting the use of topical zinc for cold sores, the magnitude of benefit appears limited, and results are inconsistent. Oral zinc supplementation is generally not recommended for this purpose unless a deficiency is present. Most clinical guidelines do not list zinc as a primary therapy for cold sores, but it may be considered as an adjunctive option. The evidence rating is therefore moderate to low (2/5), reflecting limited but present scientific validation rather than tradition alone.
