Magnesium is widely recognized for its essential role in cardiovascular health, with significant scientific evidence supporting its use in the prevention and management of certain cardiovascular diseases (CVD). Magnesium acts as a natural calcium channel blocker, influences vascular tone, and is crucial for normal cardiac rhythm. Epidemiological studies have demonstrated that low serum magnesium levels are associated with increased risk of hypertension, arrhythmias, atherosclerosis, and sudden cardiac death. Randomized controlled trials (RCTs) have shown that magnesium supplementation can lower blood pressure modestly in individuals with hypertension and improve outcomes in patients with acute myocardial infarction, particularly when administered intravenously post-infarction. Furthermore, magnesium has been used effectively to manage torsades de pointes, a specific type of ventricular arrhythmia.

Despite these benefits, the overall effect size of magnesium supplementation in reducing major cardiovascular events remains moderate, and benefits are most pronounced in populations with magnesium deficiency or increased risk of deficiency (e.g., those with diabetes or diuretic use). The American Heart Association and other guidelines acknowledge the role of magnesium in arrhythmia management, although routine supplementation for all individuals with CVD is not universally recommended. Overall, the use of magnesium in cardiovascular disease is supported by substantial scientific evidence, especially for specific indications such as arrhythmia management and in patients with documented hypomagnesemia.
