Synopsis

Source of validity: Scientific
Rating (out of 5): 2

Vitamin C (ascorbic acid) has some scientific rationale for use in supporting or treating canker sores (aphthous ulcers), though the evidence is limited. Several small studies and case reports have suggested that vitamin C deficiency may be associated with an increased risk of developing mouth ulcers, and that supplementation might help reduce symptoms or frequency, particularly in individuals who are deficient. For example, a few studies have shown improvement in oral ulcer healing and symptom reduction with vitamin C administration, but most of these studies are small, uncontrolled, or involve patients with clear deficiency states (such as scurvy).

The putative mechanism involves vitamin C's role in collagen synthesis and wound healing, as well as its antioxidant properties, which may help reduce inflammation and promote mucosal repair. However, data from large, well-controlled clinical trials in people with recurrent aphthous ulcers and normal vitamin C status are lacking. Reviews and guidelines generally acknowledge a possible benefit in cases of deficiency but do not strongly recommend vitamin C for all patients with canker sores.

In summary, the scientific evidence for vitamin C's use in canker sores is limited and primarily supports its use in patients with low vitamin C status. There is insufficient high-quality evidence to recommend routine supplementation for canker sore prevention or treatment in otherwise healthy individuals.
