Vitamin C (ascorbic acid) has some scientific support for its use in the management of canker sores (aphthous ulcers), though the evidence is limited and not robust. Studies suggest that people with recurrent aphthous stomatitis (RAS) may have lower serum vitamin C levels compared to healthy individuals. Small clinical trials have evaluated the supplementation of vitamin C in individuals with canker sores. For example, a 2012 randomized controlled trial found that topical application of vitamin C reduced healing time and pain severity compared to placebo. Oral supplementation has also been studied, with some reports indicating reduced frequency and duration of ulcers, especially in individuals with a deficiency. The rationale is that vitamin C is essential for collagen synthesis and wound healing, and it also supports immune function. Deficiency in vitamin C (scurvy) is known to cause oral ulcers, so supplementation may be particularly helpful in those with suboptimal dietary intake. However, systematic reviews and large-scale randomized controlled trials are lacking, and the benefit appears to be modest at best.
