Vitamin B6 (pyridoxal) has traditionally been used in some countries, particularly in parts of Asia, as a supportive treatment for canker sores (aphthous ulcers). The rationale is based on the association of recurrent canker sores with deficiencies in several B vitamins, including B6, B12, and folic acid. However, the direct scientific evidence supporting the effectiveness of vitamin B6 supplementation specifically for treating or preventing canker sores is limited and not robust.
Most of the evidence comes from small observational studies or case reports, some of which suggest that patients with recurrent aphthous ulcers may have lower serum levels of vitamin B6. In a few small trials, supplementation with B complex vitamins (including B6) has shown some reduction in ulcer recurrence, but these studies often use combinations of multiple B vitamins, making it difficult to isolate the effect of B6 alone. There are very few randomized controlled trials, and those that exist do not provide strong evidence for a significant benefit.
