Vitamin B5, also known as calcium pantothenate, has been investigated for its potential role in supporting or treating canker sores (aphthous ulcers). The rationale stems from the broader observation that deficiencies in various B-vitamins have been associated with an increased risk of recurrent mouth ulcers. However, among the B-vitamins, the strongest evidence exists for B12, folate, and sometimes B6, rather than B5 specifically.

There is limited but suggestive scientific evidence for the use of vitamin B5 in this context. A small number of studies and case reports suggest that vitamin B5 supplementation may reduce the frequency or severity of canker sores, but these are neither numerous nor large-scale. For example, a randomized, double-blind, placebo-controlled study published in 1997 (Journal of Oral Pathology & Medicine) found that a combination of B-vitamins including B5 reduced healing time of aphthous ulcers compared to placebo, but B5 alone was not isolated as the active component. Another small trial from the 1970s reported benefit with B5, but methodological limitations reduce its reliability.

Overall, while there is some scientific basis for the use of vitamin B5 in the management of canker sores, the evidence is weak and not definitive. Its use is sometimes supported in the context of general B-complex supplementation rather than as a targeted therapy. Well-designed, large-scale clinical trials are lacking. Therefore, the evidence rating for B5 in this context is low (2/5).
