Terpenoids, also known as isoprenoids, are a large and diverse class of naturally occurring organic chemicals derived from terpenes. Their use for canker sores (aphthous ulcers) is primarily rooted in traditional and folk medicine, particularly in herbal remedies and essential oils. Historically, certain plants rich in terpenoids, such as chamomile (Matricaria recutita), myrrh (Commiphora molmol), and tea tree (Melaleuca alternifolia), have been used topically to soothe oral mucosal irritations, including canker sores. These traditional uses are often based on the perceived anti-inflammatory, antimicrobial, and wound-healing properties attributed to terpenoid compounds within these plants. However, direct scientific evidence supporting the efficacy of terpenoids specifically for canker sores is limited. Some in vitro and animal studies suggest that certain terpenoids exhibit anti-inflammatory and antimicrobial effects, which could theoretically benefit the healing of oral ulcers. For example, chamomile extract (containing terpenoids like bisabolol) has been studied in small clinical trials with modest improvements in oral ulcer healing. Nevertheless, robust, high-quality clinical trials are lacking, and most recommendations for using terpenoid-rich preparations for canker sores remain based on tradition rather than strong scientific validation. As such, while the use of terpenoids for canker sores is supported by traditional practices, scientific evidence is limited and more research is needed to confirm their efficacy and safety for this specific condition.
