Eicosapentaenoic acid (EPA), an omega-3 polyunsaturated fatty acid primarily found in fish oil, has scientific support for its use in managing some side effects associated with cancer treatment, particularly cancer cachexia (a syndrome characterized by weight loss, muscle wasting, and inflammation). Several randomized controlled trials and meta-analyses have investigated the effects of EPA supplementation in patients undergoing chemotherapy or suffering from cancer-induced cachexia. Evidence suggests that EPA may help stabilize or improve body weight and lean body mass, likely due to its anti-inflammatory properties and its ability to modulate metabolic pathways involved in muscle catabolism. Additionally, EPA has been shown in some studies to reduce inflammatory markers and may improve appetite and quality of life in cancer patients. However, results across studies are heterogeneous, with some trials showing significant benefits and others reporting minimal or no effect. The American Society of Clinical Oncology and other professional bodies acknowledge the potential role of EPA-enriched nutritional supplements in selected patients, especially those at risk for or experiencing cachexia, but do not recommend routine use for all cancer patients. Overall, while the evidence base is substantial and growing, it is not definitive, which is why the rating is moderate (3/5). Further large-scale, well-designed clinical trials are needed to clarify the populations most likely to benefit and to refine dosing regimens.
