Vitamin D has been studied for its potential role in cancer prevention and support, based on its biological effects in cell growth regulation, immune modulation, and inflammation reduction. Epidemiological studies have observed that populations with higher sun exposure, which increases vitamin D synthesis in the skin, tend to have lower rates of certain cancers, particularly colorectal, breast, and prostate cancers. Laboratory studies show that the active form of vitamin D (calcitriol) can inhibit cancer cell proliferation and induce cancer cell apoptosis (programmed cell death) in vitro.

However, clinical trial evidence remains mixed and does not strongly support a clear benefit for cancer prevention. Large randomized controlled trials, such as the VITAL study (published in 2019 in the New England Journal of Medicine), found that vitamin D supplementation did not significantly reduce the incidence of invasive cancer compared to placebo. Some subgroup analyses and meta-analyses suggest a possible modest reduction in cancer mortality, but not incidence, with vitamin D supplementation, particularly in those who are deficient.

Overall, while there is a scientific rationale and some observational data supporting a potential role, robust clinical trial evidence for vitamin D as a preventive or supportive therapy for cancer is limited. Vitamin D supplementation is currently not recommended solely for cancer prevention, but maintaining adequate vitamin D status is important for general health.
