Pineapple is used to support the healing of bruises primarily due to its content of the enzyme bromelain. Bromelain is a mixture of proteolytic enzymes found in pineapple stems and juice, and it has been studied for its anti-inflammatory and anti-edematous (reducing swelling) properties. Several clinical studies and reviews have provided evidence that bromelain, when taken orally, can help reduce swelling, bruising, and healing time after trauma or surgery. For instance, a 2012 systematic review in Evidence-Based Complementary and Alternative Medicine noted that bromelain was effective in reducing bruising and swelling in patients after surgical procedures, such as rhinoplasty. The proposed mechanism is that bromelain helps break down fibrin and modulates inflammatory mediators, which can speed up the resolution of bruises and edema.

However, most studies use purified bromelain supplements at specific dosages, rather than pineapple fruit itself, since the amount of bromelain in fresh pineapple is significantly lower and may be destroyed during digestion. While pineapple has a long history of traditional use in folk medicine for inflammation and wound healing, modern scientific validation focuses on isolated bromelain extracts.
