Allicin, a sulfur-containing compound derived from garlic (Allium sativum), has a long history of traditional use in folk medicine for treating skin infections, including boils (furuncles). The rationale for its use is primarily based on garlic's broad-spectrum antimicrobial properties, which have been recognized in various cultures for centuries. Traditionally, crushed garlic has been applied topically to boils to help draw out infection and reduce inflammation. While laboratory studies have demonstrated that allicin exhibits antibacterial activity against Staphylococcus aureus—a common causative agent of boils—there is a lack of robust clinical trials directly evaluating the effectiveness of allicin or garlic preparations for the treatment of boils in humans. Most evidence remains anecdotal or derived from in vitro studies showing that allicin can inhibit the growth of several bacteria. Consequently, the use of allicin for boils is mainly supported by traditional medicine, with some biological plausibility based on laboratory findings, but without strong clinical validation. Caution is also advised, as topical application of raw garlic can cause chemical burns or allergic reactions in some individuals. Thus, while tradition supports its use and there is some preliminary scientific rationale, rigorous human studies are lacking.
