Serrapeptase is a proteolytic enzyme originally derived from the silkworm. Its use in supporting or treating backache is primarily based on its proposed anti-inflammatory and analgesic effects. There is some scientific evidence—though limited and of low to moderate quality—suggesting that serrapeptase may help reduce pain and inflammation associated with various conditions, including musculoskeletal pain. A few small clinical trials and animal studies have reported modest reductions in swelling, pain, and inflammation when serrapeptase is used, often compared to placebo or nonsteroidal anti-inflammatory drugs (NSAIDs). However, these studies generally include mixed pain populations (e.g., dental, postoperative, or traumatic pain) rather than specifically chronic or non-specific backache.

There is a lack of large, well-designed randomized controlled trials specifically targeting backache, and the mechanistic data are limited. Most evidence for serrapeptase’s use in backache is extrapolated from its anti-inflammatory activity observed in other types of soft tissue injuries. The use of serrapeptase for backache is not based on traditional medicine practices, but rather on more recent scientific hypotheses and limited clinical data. Overall, the evidence is preliminary, and more rigorous studies are needed to determine its efficacy and safety for back pain.
