Blue Flag (Iris versicolor) has a history of use in North American traditional herbal medicine, particularly among Indigenous peoples and 19th-century eclectic physicians. It was reputedly used for a variety of ailments, including as a remedy for backache, rheumatism, and as a "blood purifier." References to its use for back pain are largely anecdotal and stem from traditional herbal texts rather than scientific studies. The plant’s rhizome was thought to have anti-inflammatory and analgesic properties, and decoctions or tinctures were sometimes applied topically or taken internally for musculoskeletal complaints.

However, there is a lack of modern clinical research evaluating Blue Flag’s efficacy or safety for backache or related conditions. No controlled human studies exist to support its use for pain relief, and its active constituents (such as iridin) have not been conclusively shown to possess significant analgesic or anti-inflammatory effects relevant to back pain. Furthermore, Blue Flag is known to be potentially toxic if not properly prepared, and may cause gastrointestinal upset or more serious adverse effects. As such, its use for backache is based primarily on historical tradition rather than scientific validation. Modern herbal and medical sources generally do not recommend Blue Flag for this purpose, and its use has largely fallen out of favor.
