Aconite (derived from plants in the Aconitum genus, also known as monkshood or wolf’s bane) has a long history of use in traditional medicine, particularly in Chinese and Ayurvedic systems, for the management of pain, including backache. Traditional Chinese Medicine (TCM) practitioners have used processed aconite roots (often as Fuzi) for centuries to treat various forms of musculoskeletal pain, citing its warming and analgesic properties. However, the use is rooted in traditional beliefs about its ability to dispel "cold" and relieve pain rather than robust clinical evidence.

From a scientific standpoint, there is minimal high-quality evidence supporting the efficacy or safety of aconite for backache. Most modern studies focus on its toxicity rather than its therapeutic benefits, as aconite contains potent alkaloids (like aconitine) that can be highly toxic and even fatal if improperly prepared. While some laboratory and animal studies suggest that aconite alkaloids have analgesic and anti-inflammatory properties, clinical trials in humans are rare, small in scale, and often methodologically weak. Thus, while aconite's use for backache is well documented in traditional systems, scientific validation is lacking, and safety concerns are significant.
