Capsicum, particularly in the form of capsaicin (the active compound in chili peppers), has a scientifically validated role in the management of arthritis, especially for alleviating pain associated with osteoarthritis and rheumatoid arthritis. Capsaicin works by depleting substance P, a neuropeptide involved in transmitting pain signals to the brain. Topical capsaicin creams and patches are commonly recommended and have been studied in clinical trials. A number of randomized, double-blind, placebo-controlled studies have shown that topical capsaicin can reduce pain when applied to the affected joints, often with moderate efficacy and minimal systemic side effects. Meta-analyses and guidelines from organizations such as the Arthritis Foundation and the American College of Rheumatology support the use of capsaicin cream as an adjunctive therapy for some patients with arthritis pain. However, oral consumption of capsicum or capsaicin for arthritis is less well-supported and may not offer the same benefits as topical application. Side effects can include local irritation or burning sensations, but these usually diminish with continued use. Thus, while capsaicin is not a cure for arthritis, its topical use is scientifically justified as a pain management option, supported by moderate-quality evidence and clinical guidelines.
