Andrographis paniculata, commonly known as "King of Bitters," has been used in traditional medicine systems such as Ayurveda and Traditional Chinese Medicine for its anti-inflammatory and immune-modulating properties. More recently, there has been scientific interest in its potential to support or treat arthritis, particularly due to its primary active compound, andrographolide. Several preclinical studies (in vitro and in vivo) have demonstrated that andrographolide can inhibit the production of pro-inflammatory cytokines and mediators involved in the pathogenesis of arthritis, such as tumor necrosis factor-alpha (TNF-α) and interleukin-1β (IL-1β). Animal models of rheumatoid arthritis have shown that Andrographis extracts can reduce joint swelling and tissue damage.

A few small randomized controlled trials and open-label studies in humans suggest that Andrographis, alone or in combination with other herbs, may help reduce symptoms in rheumatoid arthritis and osteoarthritis, such as pain and joint stiffness. However, these studies are generally limited by small sample sizes, short duration, and methodological weaknesses. Systematic reviews note that while preclinical data is encouraging, there is insufficient high-quality clinical evidence to recommend Andrographis as a stand-alone treatment for arthritis. More robust, large-scale clinical trials are needed to confirm efficacy and safety.

In summary, the use of Andrographis for arthritis has some scientific basis, primarily from experimental studies, but clinical evidence remains preliminary and not definitive.
