Nisin is a bacteriocin, a type of antimicrobial peptide produced by Lactococcus lactis. It has been extensively used as a food preservative due to its ability to inhibit the growth of Gram-positive bacteria, including some antibiotic-resistant pathogens such as Staphylococcus aureus and Clostridium difficile. Scientific research has shown that nisin disrupts bacterial cell walls by binding to lipid II, a key component in cell wall synthesis, leading to cell death. Several in vitro and animal studies have demonstrated that nisin can reduce bacterial load, and it is being explored as an alternative or adjunct to traditional antibiotics, especially in the context of rising antibiotic resistance.

However, while there is compelling preclinical evidence for its antimicrobial activity, clinical studies in humans are limited. Nisin is not currently used as a systemic antibiotic in clinical medicine; its primary applications remain in food preservation and, to a lesser extent, topical or oral antimicrobial formulations. The evidence base for using nisin as a direct alternative to antibiotics in the treatment of human infections is still emerging, hence a moderate evidence rating. Further research, particularly clinical trials, is needed to fully establish its effectiveness and safety as an antibiotic alternative.
