Vitamin B12 is used to support or treat Alzheimer's Disease primarily based on scientific reasoning, though the strength of evidence is limited. B12 deficiency can cause neurological symptoms resembling dementia, including memory loss, confusion, and cognitive decline. Correction of deficiency with B12 supplementation can reverse these symptoms in deficient individuals. Epidemiological studies have found that low B12 levels are more common in older adults and may be associated with an increased risk of cognitive decline and dementia, including Alzheimer's Disease. Additionally, B12 plays a role in homocysteine metabolism; elevated homocysteine is a risk factor for cognitive impairment and Alzheimer's, and B12 supplementation can help lower homocysteine levels.

However, controlled clinical trials in people with Alzheimer's Disease or mild cognitive impairment (MCI) have not consistently shown that B12 supplementation improves cognitive function in those without clear deficiency. Most guidelines suggest screening for and correcting B12 deficiency in patients with cognitive impairment, but do not recommend B12 supplementation for all Alzheimer's patients. The evidence is strongest for individuals who are deficient in B12, while routine use as a treatment for Alzheimer's in those with normal B12 levels is not supported by robust evidence. Thus, while there is a scientific rationale and limited evidence supporting B12 supplementation in deficient individuals, its role as a general treatment for Alzheimer's Disease is not well established.
