Caprylic acid (octanoic acid) is a medium-chain triglyceride (MCT) that has garnered interest for its potential role in supporting cognitive function in Alzheimer's Disease (AD). The rationale stems from the observation that individuals with AD exhibit impaired glucose metabolism in the brain. MCTs like caprylic acid are rapidly metabolized in the liver to produce ketone bodies, which can serve as an alternative energy source for neurons when glucose metabolism is compromised. This "ketogenic" strategy aims to bypass the energy deficit believed to contribute to cognitive decline in AD patients.

Evidence supporting the use of caprylic acid is limited but does have a scientific basis. Some small clinical studies and pilot trials have tested MCT formulations (e.g., Axona®, which contains caprylic triglyceride) in patients with mild to moderate AD. These trials report modest improvements in cognitive performance in certain subgroups, particularly those without the APOE4 genetic variant. However, the data are not robust, and larger, well-controlled studies are lacking. Systematic reviews and meta-analyses generally conclude that while the biological mechanism is plausible, the clinical benefits are uncertain and likely to be modest at best.

In summary, there is a scientific rationale and preliminary evidence for caprylic acid/MCTs in AD, but the overall level of evidence remains low (rated 2/5). It should not be considered a primary or stand-alone therapy, and any use should be under medical supervision.
