Berberine is a plant alkaloid found in several medicinal herbs, including Berberis vulgaris (barberry) and Coptis chinensis (goldthread). In recent years, it has attracted scientific interest for its potential neuroprotective properties, including possible benefits in Alzheimer's disease (AD). Experimental studies (primarily in vitro and in animal models) have suggested that berberine may exert multiple mechanisms relevant to AD pathology: it can inhibit acetylcholinesterase (increasing acetylcholine levels), reduce amyloid-beta aggregation, attenuate oxidative stress, and decrease neuroinflammation. For instance, a 2015 study in rats showed that berberine administration improved cognitive performance and reduced amyloid plaque burden. Other studies have found that berberine can modulate signaling pathways involved in neurodegeneration, such as the PI3K/Akt and AMPK pathways.

However, there is only limited evidence from human studies. Most clinical research on berberine focuses on metabolic disorders (such as diabetes and hyperlipidemia), not Alzheimer's disease. There are currently no large, high-quality randomized controlled trials in humans directly evaluating berberine for AD. Thus, while preclinical data are promising and suggest a scientific rationale, the evidence base is still early and insufficient to recommend berberine as a treatment for Alzheimer's disease. More rigorous clinical research is needed to validate its effectiveness and safety in this context.
