Magnesium is scientifically used in certain cases to support or treat alkalosis, particularly when hypomagnesemia (low magnesium levels) is present alongside metabolic alkalosis. Metabolic alkalosis can occur due to excessive loss of hydrogen ions (such as from vomiting or diuretic use), and is often associated with concurrent deficiencies in potassium and magnesium. Magnesium plays a crucial role in maintaining cellular ionic balance and is a cofactor for many enzymes involved in acid-base homeostasis. Hypomagnesemia can worsen or perpetuate hypokalemia, which in turn can make correction of alkalosis more difficult. Therefore, repletion of magnesium is considered an important adjunct in the management of alkalosis when deficiency is documented or suspected.
There is scientific evidence from clinical practice guidelines and textbooks that recommend correction of magnesium in alkalotic patients with confirmed hypomagnesemia (e.g., Goldman’s Cecil Medicine, 25th Edition, and UpToDate on metabolic alkalosis management). However, magnesium supplementation is not used to "treat" alkalosis directly in the absence of deficiency; its role is supportive, helping to correct electrolyte disturbances that are often intertwined with the pathophysiology of alkalosis. Randomized controlled trials specifically targeting magnesium for alkalosis are lacking, but the practice is well established in clinical medicine based on pathophysiological rationale and observational data.
In summary, magnesium is not a primary therapy for alkalosis, but its supplementation is scientifically justified and routinely recommended when hypomagnesemia coexists with alkalosis.
