Vitamin C (ascorbic acid) has some scientific support for use in acne management, though the evidence is not robust. Its primary mechanisms of action in acne are related to its antioxidant, anti-inflammatory, and skin-healing properties. Oxidative stress and inflammation play roles in acne pathogenesis, and vitamin C can reduce oxidative damage by neutralizing free radicals. It also inhibits NF-κB, a key regulator of inflammation, which may help reduce redness and swelling in acne lesions.

Topical vitamin C may help lighten post-inflammatory hyperpigmentation (PIH) that often follows acne, by inhibiting melanin synthesis. Clinical studies have shown that topical vitamin C serums can improve skin tone and reduce PIH, though its direct effect on active acne lesions is less clear. Some small studies have also indicated that topical vitamin C, alone or in combination with other agents (such as niacinamide or zinc), may reduce the number and severity of acne lesions, but these studies are limited in size and quality.

Oral vitamin C supplementation is not well-supported for acne treatment, as there is little evidence it meaningfully impacts acne development or severity. Topical formulations are generally considered safe and may serve as an adjunct to established acne therapies but should not replace first-line treatments like retinoids or benzoyl peroxide. In summary, while there is some scientific rationale and preliminary evidence for vitamin C use in acne (especially for PIH), large, high-quality clinical trials are lacking, and its efficacy for active acne remains modest.
